For that All Teachers are informed to fill the form and submit their data. All measures should be taken to fill data correctly.

READ THE INSTRUCTIONS CAREFULLY
- The form is in Two Pages
- Fill All the Columns applicable to you in both the Pages
- Leave the fields BLANK, which are not applicable to you
- All Dates must be filled in "dd/mm/yyyy" format. Example: Date of Birth as: 21/02/1980
- You can take the help of Calender on the form (Click on the Symbol) to select the exact date
- After submitting the form, go to Next page and follow the onscreen instructions given in the page
- Take Print and submit TWO copies to your MEO/Gr.II HM duly signed and verified
- If you are unable to submit or take print of the application, due to power failure or any other problems, no need to worry. You can check the status of Your submitted appliction using your Treasury Id Number
